On your first visit, please arrive approximately 15 minutes prior to you scheduled appointment time to allow time to complete the applicable healthcare forms. If you prefer, you may print out and complete the forms above at home and bring them into the office on your first visit. After completing the forms, you will meet with Dr. Dailey to discuss your symptoms and undergo a detailed examination (if necessary, x-rays will be taken at this time).   Please be sure to bring your driver’s license and health insurance card, if applicable.

A follow-up appointment will be set where Dr. Dailey will go over the findings of your examination and x-rays and develop a treatment plan that works for you.
